Team Leader and Support Workers x 2

We are looking for a Team Leader and 2 Support Workers for our new Combat Isolation Service to assist our clients to live life to their fullest potential.

Combat Isolation is a brand new bespoke service based across Liverpool, Sefton and Kirkby. This new Team will support older adults (over 50) with complex functional mental illness who have been affected by loneliness and isolation exacerbated by the impact of Covid. Delivering one to one support, together with community based social interventions; supporting individuals, building personalised community support programs designed to supplement existing care plans and delivering activity programs/volunteer befriending services.
